This Healthy Living Privacy Notice supplements the Stanford University Online Privacy Policy. It applies to the personal information that Stanford University’s Healthy Living Program collects and uses when individuals access the Healthy Living website (https://healthyliving.stanford.edu) or participate in the Healthy Living offerings. This notice describes how this information is processed and the choices that are available to you regarding the use of the personal information you provide.

Information Collected and Used


Personal information is any information related to an identified or identifiable individual. We are committed to protecting your privacy, and we will only use the personal information that you provide to offer you resources to improve your health, for the strategic planning, promotion, administration, and evaluation of our wellness programs and benefits offerings, and to gain an understanding of the well-being of our participant population.

Healthy Living uses university and hospital single sign-on (SSO) to authenticate employees into the website. When registering for Healthy Living offerings, we collect your Stanford affiliation, full name, email address, phone number, employee ID, and Stanford department if applicable. For administration, management, and evaluation of our wellness programs, Healthy Living may access human resources information regarding university employees, including SUNet ID, gender, date of birth (DOB), employee ID, email address, organization, business unit, department, hire date, office location, employee type, union status, and other employee information. Participants may be asked to provide credit card information to process payments for classes or workshops.

For some Healthy Living offerings, you may need to provide personal information for the purpose of delivering the class content or participating in lifestyle coaching. Individuals in lifestyle coaching will be directed to schedule their appointments and answer intake questions using third-party platforms; session notes are stored in a third-party platform. Healthy Living staff will access rosters of classes that they host in order to take attendance. Representatives from the departments that request a Healthy Living for Teams class may receive a roster with names and emails of class participants in order to take attendance. Authenticated participants who watch at least 80% of recorded or on-demand webinars will be counted as “attended.”

For Stanford University benefits-eligible employees and Stanford Hospitals and Clinics employees, participant information, such as employee name, employee ID, activity code, and activity completion date will be shared with Personify Health, the digital platform used by BeWell Programs and HealthySteps to Wellness, for the purposes of granting participation credit. When an offering is provided by an outside vendor, participants should review the privacy policies and other terms of that vendor. If you sign up for an app through Healthy Living, your name and email address may be shared with the app vendor so that you can participate in their program. If you participate in our surveys, we may collect personal information such as age, phone number, gender, demographic variables, lifestyle behaviors, attitudes, and other health information.


How We Use Your Information


We may use your personal information to administer, promote, manage, and evaluate Wellness Programs, including for the following purposes:

(a) To suggest appropriate wellness resources to you, through Stanford University, with your medical plan, or through other providers.

(b) To allow you to access the Healthy Living website and health education materials and programs.

(c) To tailor emails, health promotion surveys, and programs for you, to provide information and initiatives related to health or wellness, and to inform you of Stanford programs, services, and offerings. For these purposes, your name, email address, join date, username, employeeID, SUNet or hospital ID, location, school/business unit, department, and employee type may be sent to an outside email service, MyEmma, to send emails and track the activity of these emails.

(d) If you also participate in the BeWell Program or HealthySteps to Wellness, your personal information is used to issue monetary or other rewards and to track wellness program participation.

(e) To solicit feedback about the Healthy Living Programs, evaluate the effectiveness of behavior change and health promotion programs, and suggest and plan future health programming and benefits design at Stanford University.

(f) To understand the health and well-being of our participant population as a whole, as well as subgroups, and to understand the factors that impact well-being.

(g) To understand factors that affect program participation of the population as a whole, as well as subgroups, and to understand the factors that affect and improve participation in our programs.

(h) To provide you with surveys to evaluate our offerings.

Your personal information is only used for the intended purposes as described above and only collected and processed based on a legitimate purpose and in compliance with legal and regulatory obligations. Your consent will be requested before we process your personal information for any additional purposes not described in this Privacy Notice.


Sharing Your Personal Information


We may engage third parties to process your Personal Information on our behalf for the purposes stated above.

Stanford University does not sell any Personal Information, including Personal Information of minors, to third parties for their use in direct marketing, advertising, or promotion of their products or services.


Access and Data Transfer


Except as provided herein, Healthy Living will not share your personal information with third parties unless you have consented. We only share your personal information with third parties in connection with our programs and activities. If you are a benefits-eligible University employee, Personify Health will collect and only process your personal information for the purposes described in the BeWell Privacy Notice. If you are a hospital employee, Personify Health will collect and process your personal information for the purposes of granting participation credit. If you are located outside the US and you access and use the Healthy Living website, then your personal information is transferred to the US. Please see Stanford’s Online Privacy Policy on how we protect your personal information and ensure lawful grounds are used for the transfer of personal information to the US.


Minor Information


The Healthy Living website is not intended for use by persons under the age of 18. Healthy Living does not knowingly collect personal information from such persons through this website or Healthy Living offerings. If Healthy Living becomes aware that a person under the age of 18 has provided personal information, we will take appropriate actions to stop processing and remove their personal information.


How We Protect Your Personal Information


The Healthy Living website contains links to programs and websites outside Stanford. When following a link to an external website, the terms of that website’s Privacy Notice and Terms of Use will govern the use of your personal information.


Your Choices


Your choice to use the Healthy Living website and participate in Healthy Living activities is completely voluntary. If you choose not to use the Healthy Living website, you will be unable to participate in the Healthy Living Program.


Individual Privacy Rights


Under certain circumstances, you have rights under the data protection laws that apply to you in relation to your Personal Information. Please refer to the Stanford University Online Privacy Policy. To exercise your rights, please contact healthyliving@stanford.edu.


Reviewing and Updating Your Personal Information


You may submit any questions or concerns to the Healthy Living team by emailing healthyliving@stanford.edu. Please do not include any sensitive information or personal health information in an email.


Changes to This Privacy Notice


We may modify this Privacy Notice from time to time. When we make substantive changes to this statement, the latest update will be included on our website at https://healthyliving.stanford.edu/classes/register/privacy.php. Your further use of our Services after a change to our Privacy Notice will be subject to the updated statement. If you don’t agree with the changes, then you can always stop using our services, delete your account, and stop giving us any more personal information. Where we are required by the applicable privacy laws, we will also seek your consent to any material changes that affect how we use your personal information.
